Agreement to Terms *

Agreement Statement: By submitting this form, I, the undersigned, agree to the following terms and conditions outlined in the rental agreement for the Boys & Girls Club of Northeast Florida Banquet Hall: Rental Fee: The rental fee for the banquet hall is $1,500 for a minimum of four (4) hours, with an additional fee of $250 for each additional hour. The total rental time and associated cost will be confirmed before booking. Deposit: A non-refundable deposit of $250 is required to secure the booking. The booking is not considered final until the deposit has been received. Employee Discount: If applicable, I acknowledge the 50% discount for Boys & Girls Club of Northeast Florida employees, bringing the rental fee to $750 for a minimum of four (4) hours. The discount does not apply to additional hours. Payment: I agree to make payment via one of the accepted methods: QR code, check (made payable to Boys & Girls Club of Northeast Florida), or money order. Insurance: I understand that a Certificate of Insurance naming the Boys & Girls Club of Northeast Florida as the beneficiary must be presented prior to the event. Kitchen Access: I acknowledge that access to the kitchen is restricted and requires pre-approval from the BGC Event Director. If the kitchen is needed, I understand that failure to comply with the kitchen access policy will result in a $500 penalty. Liability: I assume full responsibility for any damages or injuries that occur during the rental period. I agree to indemnify and hold harmless the Boys & Girls Club of Northeast Florida from any claims arising from my use of the property. Return of Facility: I agree to return the facility in the same condition it was received. I understand that set-up and cleanup time must be included in the total rental period. Cancellation and Refund: I acknowledge that the deposit is non-refundable unless otherwise agreed upon in writing. I understand the booking is not final until the deposit is received. Governing Law: This agreement is governed by the laws of the State of Florida.
